MSH will use Part C funding to provide services and support currently not covered by insurance or any other Ryan White Part. As the current Part C grantee, MSH is well-positioned to manage this grant as one of the most experienced providers of HIV primary care. In 2023, Mount Sinai’s HIV program, now comprised of four clinics throughout Manhattan, served over 13,500 people, more than 8,500 of whom were PWH, representing over 6.4% of all PWH in NYC. The Comprehensive Health Program (CHP), located in the Chelsea-Clinton neighborhood in Downtown Manhattan, served nearly 2,100 Part C patients in 2023, of whom 37% are Black, 34% are Hispanic/Latinx, 68% identify as men who have sex with men (MSM), and 8% are transgender. NYC has the largest HIV epidemic in the US, with approximately 5% of all new HIV diagnoses and 11% of PWH nationwide. In NYC, it is estimated that 7% of PWH are unaware that they are infected, and once diagnosed, 18% are concurrently diagnosed with AIDS, suggesting diagnosis in late stages of disease progression. Among those who have been diagnosed, 19% are not currently engaged in care, and 47% are not virally suppressed within 3 months of diagnosis. The target populations for MSH are drawn from across the five boroughs of NYC, with the greatest number of clients coming from our immediate neighborhood of Chelsea-Clinton. The target population includes low income, medically underserved people either at high risk for or currently living with HIV. Matching trends in the HIV epidemic, the target population includes African American and Latinx PWH, men who have sex with men (MSM), young men of color who have sex with men (YMSMoC), and transgender and gender non-binary (TGNB) persons. MSH will provide high quality, co-located, comprehensive health services. In Year 1 of the project period, 3,600 PWH will be provided with HIV primary care at the MSH’s Comprehensive Health Program. In addition, 1,500 people will receive rapid HIV testing with 100% of new positives identified linked to care within 3 months. Seven hundred twenty (720) patients will receive mental health treatment; 1,080 will receive substance use treatment; 100% of all new patients will be screened for co-infection with hepatitis B; 75% of all patients will be screened for hepatitis C; and 1,440 will receive non-medical case management services.
