Ryan White Part C Outpatient EIS Program - PROJECT ABSTRACT Project Title: Crossroads Clinics North, Ryan White Program Part C EIS Applicant Name: Delta Regional Medical Center Address: 1400 East Union St. Greenville, MS 38703-3246 Project Director: Chad Neal Contact Phone Numbers: Phone (662) 332-1398 Email: cneal@deltaregional.com GENERAL OVERVIEW OF HIV EPIDEMIOLOGY There were 430 people with HIV (PWH) in our service area (Washington, Sunflower, and Bolivar County) in 2022. 395 (91.86%) were Black, 16 (3.72%) White, and 5 (1.16%) Multi-Racial. Their genders were 295 (68.60%) male and 137 (31.86%) female. 205 (47.67%) were men that have sex with men MSM). cdc.gov. Ages 55-64 accounted for the most PWH (42) in Bolivar, followed by Sunflower ages 55-64 (42) cases, and Washington ages 55-64 (41). Due to poverty, lack of insurance, and social stigma there are a high percentage of PWH in Washington, Sunflower, and Bolivar County who have difficulty accessing culturally competent and appropriate medical evaluations and clinical care. Outside of Ryan White funded facilities, there continues to be a need for ongoing, culturally competent, localized primary care available to PWH, particularly among communities of color in the service area. Washington (27.3%), Sunflower (32.3%), and Bolivar County (29.1%) represented a population that is significantly higher in poverty than the rest of the state of Mississippi (19.1%). The median household income in 2023 for the 3 counties were $37,880 compared to the state average $52,98500. www.census.gov. KEY SERVICES: Dr. Satwinder Singh, a board-certified Infectious Disease Specialist, will provide HIV primary care and act as the Medical Director of the Ryan White Part C EIS Program. The program will also provide medical and non-medical case management, on-site lab specimen collection, Tuberculosis, Hepatitis B/C, and sexually transmitted infection (STI) evaluation and treatment, risk and adherence counseling, specialty referrals, emergency financial and transportation assistance. A subgrantee agreement from the Mississippi State Department of Health Ryan White Part B services also funds HIV primary care, medical nutritional counseling, and oral health care through an on-site dental operation. 211 clients were enrolled in our program in 2023. REQUESTED: Delta HealthSystem The Medical Center, a county-owned facility that provides comprehensive inpatient and a range of outpatient services, is requesting funding for periods May 1, 2025 – April 30, 2028, Part C EIS in the amount of $282,723.00. We are requesting funding preferences under Qualification 1: Increased Burden, Qualification 2: Rural Areas, and Qualification 3: Underserved Areas. TARGET POPULATION: Our clients in care over the last three years (2021-2023) have been African American (98.17%) followed by at Whites (1.83%) made of up the rest of the overall client population. This closely resembles our services area with African Americans leading other subpopulations with the highest HIV incidence of HIV (96.88%) and prevalence (91.60%). Over this span of time, there were 17 newly diagnosed clients entering care, 16 were African American. The number of newly diagnosed individuals in our service area were 8 (2020), 15 (2021), and 9 (2022). Black MSM continues to be the most at risk subpopulation severely affected by HIV. The burden of HIV is disproportionately high for men who have sex with men (MSM). Among men whose risk factors for HIV are known, 50.27% percent of men reported having male-to-male sexual contact from 2021-2023.
