The funding of this proposal allows the Florida Department of Health in Duval County (FDOH Duval) Comprehensive Care Center (CCC) to continue to provide a full range of services to reduce HIV transmission and improve health outcomes for those living with HIV. The services offered at CCC include Ryan White HIV/AIDS Program (RWHAP) core services and support services. The target population for this existing geographic area includes: • Individuals who are newly diagnosed. • Individuals who have left the HIV service system (out of care) • Individuals who are aware they have HIV and have never sought care (aware and not in care). It has been established that our geographic area of Duval County includes PWH, those with behaviors that put them at risk of contracting HIV, and those that are living in zip codes that are most impacted by HIV and socioeconomics have a high impact on health outcomes. Over the past three years, HIV diagnoses in Duval County has increased by 27.3% between 2020 (n=231) and 2022 (n=294). In 2022, Blacks represented two out of every three reported cases by accounting for 66.9% of new HIV diagnoses. The Duval County continuum of care demonstrates that there is an unmet need of approximately 33% of PWH that do not have a suppressed viral load and approximately 18.6% of PWH not receiving care for their HIV. FDOH Duval has implemented an efficient HIV care continuum through coordinated planning and collaborative efforts. We have extended our service delivery model beyond the brick-and-mortar clinical setting through use of our mobile medical unit. FDOH Duval is intentionally taking as much of our in-house one stop shop services to our community to provide routine STI and HIV screening, linkage to care and treatment, and other services that engage, retain in care, and to move towards the final goal of Ending the HIV Epidemic.
