Ryan White Part C Outpatient EIS Program - Special Health Resources (SHR) Inc. is a nonprofit 501(c)(3) Federally Qualified Health Center and a Community Behavioral Health Clinic that is located in the Northeastern part of Texas. SHR was founded over 30 years ago as an AIDS Services Organization and today continues to be the largest HIV service provider in the area dedicated primarily to the prevention and treatment of HIV disease. It has four locations throughout the catchment area to increase access to services across the whole region. The four offices are at the following locations: 410 North Fourth Street, Longview, TX; 1300 Clinic Drive, Tyler, TX; 3150 Clarksville, Paris, TX; and 1102 Main Street, Texarkana, TX. SHR has developed a “one-stop-shop” service model that includes street outreach, HIV counseling/testing, case management, HIV early intervention services, clinical services, STD screenings, basic dental care, substance abuse treatment, mental health counseling, and transportation services. Collectively, these services establish a continuum of care for vulnerable HIV infection and HIV-infected persons living in East Texas. SHR proposes to continue providing outpatient comprehensive primary health care and support services to low-income, uninsured, and underinsured people living with HIV. SHR will continue to provide the following: 1) targeted HIV counseling, testing, and referral; 2) medical evaluation, clinical, and diagnostic services; 3) Therapeutic measures for preventing and treating conditions arising from HIV/AIDS; and 4) referrals to appropriate providers of health care and support services. The program will focus on increasing the number of persons from racial and ethnic minority populations identified with HIV infection and expanding their linkage to HIV care. The overarching goal is for primary care among persons with HIV disease to start as early in the course of the infection as possible. SHR requests funding preference for Qualification 1: Increased Burden and Qualification 2: Rural Areas funding. SHR is located in a predominantly rural area that includes counties designated by HRSA as Health Profession Shortage Areas and Medically Underserved Areas. Hence, this area has very limited primary care services in general and even more so services targeting HIV-infected persons. SHR is requesting funding for a period of three years to continue providing quality HIV primary care services in the Tyler and Texarkana Health Service Delivery Areas (HSDA) which is comprised of the following counties (includes all counties indicated in the RFP for the Longview area): Anderson, Bowie, Camp, Cass, Cherokee, Delta, Franklin, Gregg, Harrison, Henderson, Hopkins, Lamar, Marion, Morris, Panola, Rains, Red River, Rusk, Titus, Upshur, Van Zandt, and Wood. All but one of the target counties are considered full or partially rural.
