Joint Commission accredited since 1991, and in keeping with the prevailing mission to improve the health status of communities by providing an integrated system of high-quality primary and preventive health services, responded to need by providing HIV Case Management since 1997. Service Coordination for the People With HIV (PWH) and affected family members was added in 2001 through a contract with Tri-County Community Health Center’s Title IV (Part D) program, CommWell Health (CWH) and complemented by the addition of Title III (Part C) Early Intervention Services in 2003. RHHC’s numerous services and resources support PWH in all phases of the continuum of care that is needed to engage and remain in care and receive medications, primary care, and essential therapies. RHCC proposes to continue existing, integrative, preventive and primary HIV services in the designated, project service area of seven (7) counties in North Carolina: Hoke, Montgomery, Moore, Scotland, Richmond, Robeson, and Cumberland, the latter through a strong collaboration with Tri County Community Health Council (TCCHC) dba CommWell Health (CWH). The broad geographic size of this area spans 3,977 square miles, is situated in the southeast portion of the state, and is primarily rural, with an array of small cities, towns, villages, and enclaves. The largest city in this varied geographical region is Fayetteville, population 218,099, located in the also largest RW Part C service area county of Cumberland. In 2022 2,878 people were diagnosed with HIV in the RHCC service area, compared to 2,792 in 2021, the majority of which collectively were Black/African American and Native Americans, indicating the need for targeted outreach focused on BIPOC populations. RHCC is a trusted community resource, which is critical to engaging more local residents in HIV prevention and treatment. In order to enter people into care sooner, RHCC proposes a plan that will break down barriers to care (transportation, essential resources, assistance with insurance/social service program enrollments), and provide supportive local services to enhance community connections. RHCC’s care coordination, integrated behavioral health and primary care providers, and patient care navigation programs are ideal to address the social determinants, mental, and physical health needs to improve HIV outcomes. Robeson Health Care Corporation (RHCC) requests an increase in the commitment of resources to enroll, retain, and address suppression and core continuum care needs in response to an additional number of People With HIV (PWH) residing in a high-need, under-resourced area of North Carolina. In addition to existing, unmet need in the service area, implementation of the Center for Disease Control and Prevention (CDC) recommendations for standard HIV testing will increase the demand for the core continuum of care services, as residents in the service area are newly diagnosed and identified as HIV positive.
