Ampla Health has a comprehensive program with key services that supports all aspects of primary care, specialty referrals, medical and nonmedical case management, and substance use disorder, through collaborative efforts with area HIV services programs, public health departments, and home-health organizations. Ampla Health is requesting the ceiling amount of $406,812 to continue our Ryan White HIV/AIDS EIS Program throughout our 19-area clinics within our 6county service area. A "Funding preference" is requested as a qualified applicant in all areas. Target populations include, but are not limited to individuals experiencing homelessness, substance use disorders, mental health disorders, poverty, language barriers (i.e., social determinants), medically underinsured, uninsured, persons living with disabilities, persons living with HIV/AIDS, and their household members. In our current, active patient population (N= 350), 166 patients (47.43%) a below 100% of the Federal poverty level, and over 60% reside in Butte County - an area stricken with destructive wildfires, drought, flooding, high substance and alcohol use. With continued funding, Ampla Health's Ryan White Part C EIS program intends to serve PLWHA in our patient catchment areas, strive to end the HIV epidemic by 2030, retain patients in care to suppress viral load, link patients to care, reduce infectious disease, educate, and improve quality of life and lifespan for people living with HIV/AIDS (PLWHA). Ampla Health will provide to PLWHA: 1. HIV screening and education 2. HIV primary care and treatment 3. Mental health and substance use disorder counseling 4. Care for aging adults, and the deterioration of the immune system for conditions arising from HIV/AIDS. 5. Offer referrals to appropriate providers of health care and support services. Such services will be provided by in-person encounters, telehealth visits, or telephonically. Ampla Health's aim is to continue to provide quality health care to PLWHA in northern California's rural areas
