Ryan White Part C Outpatient EIS Program - Sixteenth Street Community Health Centers (SSCHC) is a Federally Qualified Community Health Center and has provided services to Milwaukee’s south side residents for over 55 years. The mission of SSCHC is to improve the health and well-being of Milwaukee and surrounding communities by providing quality, patient-centered, family-based health care, health education and social services, free from linguistic, cultural, and economic barriers. The Ryan White Part C Early Intervention Services Program will provide medical care and case management services to People with HIV (PLWH) on Milwaukee’s near south side, complimenting SSCHC’s mission. Our primary service area, as listed in Appendix B of NOFO HRSA-25-003, is comprised of zip codes 53204, 53207 and 53215. The HIV services currently available in our service area include counseling, testing, referral services, partner notification, transportation, relocation assistance, food pantry, and legal aid, childcare, and HIV+ case management services. The HIV services provided specifically by SSCHC under the Ryan White HIV/AIDS Program include early intervention services (counseling, testing, referral services for individuals at high risk of HIV infection and linkage to care for PLWH), medical case management including treatment adherence education, oral health care, outpatient ambulatory health services, substance abuse outpatient care, linguistic services, medical transportation, non-medical case management services, outreach services, referral to health care and support services, and psychosocial support services. SSCHC also incorporates mental health services into each participant’s general HIV/AIDS treatment plan to ensure state-of-the-art HIV care. SSCHC provides comprehensive primary medical and behavioral health care in English and Spanish. The funding requested is $372,403.00. SSCHC’s efforts focus to reach primarily Black, Indigenous and other People of Color BIPOC with special emphasis in the Hispanic community. The near south side of Milwaukee, in which SSCHC primarily serves, has been identified as a medically underserved and health care professional shortage area. It has high rates of poverty, crime, gang activity, degraded housing stock and environmental hazards. The number of PLWH in our service area reported by the Epidemiology Department from the WI DHS Communicable Disease and Harm Reduction Division for CY 2023 was 550, with SSCHC reaching almost 50% of PLWH in our service area and about 40% of those who identified as Hispanic. Despite the challenges resulting from the COVID-19 pandemic in 2020, SSCHC leveraged telehealth and longstanding relationships to provide services to 256 PLWH. Due to the positive impact that telehealth has on the population that SSCHC serves we have continued to provide telehealth services. SSCHC has experienced an increase in migration of people from other states and countries. New immigrants with high needs often have knowledge deficit of health literacy/navigating the healthcare industry. Funding through this grant will allow SSCHC to continue to provide lifelong, comprehensive, bilingual, in-house, primary care and supportive services to PLWH, regardless of ability to pay, in order to increase adherence to antiretroviral therapy (ART), improve health outcomes and general quality of life contributing to efforts to achieve the End of the HIV Epidemic. These services in coordination with referrals for specialty care will also address the health issues that people aging with HIV face, including comorbidities and other health conditions that come with age.
