Because of this seven-county region's medically underserved and rural nature, local patients living with HIV/AIDS increasingly rely on comprehensive care provided by Hudson Headwaters to meet their treatment needs. Hudson Headwaters does not receive any additional Ryan White funding outside of the Part C EIS Program. Hudson Headwaters, a not-for-profit 501c3 organization, has served the Adirondack and North Country regions of New York as a Federally Qualified Community Health Center since 1981. Its service area is large, over 7,900 square miles, with a population of 526,978. It includes all or parts of Clinton, Essex, Franklin, Hamilton, Saratoga, Warren, and Washington Counties. An estimated 769 persons in the region are living with HIV/AIDS. Still, this number is likely an underestimate due to fear, stigma, and concerns about disclosure and financial need among at-risk populations. Populations targeted by this project include men who have sex with men (MSM), African Americans, Latinx, homeless, migrant workers, post-incarcerated persons, and those using illicit substances. This project will expand testing and reduce barriers to treatment to ensure access to quality, affordable care for at-risk and HIV-infected individuals. The project's primary goal is to assure a coordinated continuum of HIV care, including improved diagnosis, linkage to care, retention in care, and successful antiretroviral treatment for those living with HIV/AIDS in the service area. Client-centric services to be provided include screening and prevention activities among high-risk populations, comprehensive primary care services, mental health services, enhanced medical care management and support of patients' efforts to adhere to treatment (i.e., antiretroviral medications and substance use disorder treatment), and education and self-management. In addition, Hudson Headwaters' Ryan White Program will provide clinical education programs for HIV providers, employ a peer leader who provides valuable advocacy, coaching, and outreach to clients, run Clinical Quality Management (CQM) Committee meetings to review data and discuss improvement, and conduct quality improvement (QI) projects that focus on enhancing the quality of Care for all HIV/AIDS patients.
