CareSouth Carolina, Inc. (CSC) is a federally qualified community health center (FQHC) that provides primary, preventive, and integrated behavioral health care for medically underserved residents of the northeastern Pee Dee region of South Carolina (SC). Established in 1980 in Society Hill, CSC is one of the few providers in the region that accepts all Medicaid and Medicare patients and offers a sliding fee scale for the uninsured. In 2001, CSC implemented its Care Innovations division, delivering comprehensive Ryan White (RW) Part C Early Intervention Services (EIS), including outpatient primary health care and support services, to people with HIV/AIDS (PWH) in Chesterfield, Darlington, Lee, and Marlboro counties, SC. In accordance with the Ending the HIV Epidemic initiative, CSC’s Part C EIS program strives to decrease transmission, improve health outcomes for PWH, and reduce HIV-related health disparities and inequities by focusing on the priority strategies of Diagnose, Treat, Prevent, and Respond. Continuation of funding will ensure that CSC can continue providing convenient and accessible comprehensive primary care to low-income PWH in this region, ensuring access to the essential HIV care, treatment, and support services needed to help them achieve viral suppression. Service Area and Needs. As of December 2022, there were 628 PWH in the CSC service area. This marks a 3.5% increase (from 607) since 2020. There were 283 people, or 45.1%, living with HIV-non-AIDS, while 54.9% (345) were people living with AIDS. The number of people newly diagnosed with HIV remained flat at 17 new diagnoses in 2020 and 2022, while the number people of newly diagnosed with AIDS was also unchanged with nine new diagnoses in 2020 and 2022. Access to the EIS services offered at CSC may help limit the transmission of HIV in the community and improve outcomes for PWH, as well as slow the progression of the disease among those who are HIV positive. Proposed Project, including Key Services, Amount Requested, and Target Populations. CSC requests RW Part C funding to continue providing Part C services using an interdisciplinary team targeting at-risk and PWH in their progression along the HIV Care Continuum. Services are designed to address local gaps and barriers and include: comprehensive, culturally competent primary medical care and infectious disease services; medical case management; treatment adherence services; pharmaceutical assistance; medical nutrition therapy; outreach; onsite mental health services; oral health services; health education; and linkages to referral services such as specialty care. In 2023, the CSC RW Part C program served 232 patients, an increase from 201 patients in 2020. The majority (83.6%) of the RW patients served by CSC were African American. Many (58.6%) of HIV/AIDS patients report contracting the disease through heterosexual contact, while 41.4% have transmission attributed to MSM (RSR).
