Ryan White Part C Outpatient EIS Program - Thundermist Health Center (THC) has been in operation since 1969. Its mission is to improve the health of our patients and communities by delivering exceptional health care, removing barriers to that care, and promoting healthy lifestyles. THC serves three communities, Woonsocket, West Warwick, and Wakefield, RI. THC is an NCQA - Recognized Level 3 Patient Centered Medical Home and has been providing integrated care to meet the needs of all patients. In 2023, THC served over 62,000 patients. THC serves patients across the lifespan, from infants to seniors. THC’s patients are primarily members of medically underserved populations and communities often with intergenerational trauma and medical complexity. THC has been a successful Ryan White recipient since 2002. In 2023, the Ryan White Part C Program provided care to 218 unduplicated patients. Of these, nearly 75% of them received at least one core medical service. Among the 218 unique patients who received outpatient primary care services in 2023, the following demographics were identified: By gender, females account for 29.8%, males account for 62.4%, and transgender account for 3.2% of patients served. By age, 79.8% were 43 years or older. By race/ethnicity, 22.5% identify as Latino, 17.9% identify as Black/African American, and 48.6% identify as White. By transmission mode, 42% identify as MSM, 12.6% identify as IDU, and 48.3% identify as heterosexual. THC seeks $335,296 to continue to deliver the highest quality HIV outpatient primary health care to People Living with HIV (PLWH) in Northern Rhode Island (NRI), the primary target area for the intervention. THC will provide outpatient/HIV ambulatory medical care, behavioral health services (individual counseling and medication management), medical case management, oral health services, adherence counseling, and HIV counseling and testing for those at highest risk for HIV in the target area. Target populations for PLWH include females, transgender individuals, men who have sex with men (MSM), injection drug users (IDU), Black/African Americans, and Latinos. THC’s robust quality improvement plan includes the tracking and management of all HAB suggested measures. The primary “core” measures we track are viral load suppression <200, prescription of HIV antiretroviral therapy, HIV medical visit frequency, gap in HIV medical visits, and PCP prophylaxis.
