This represents 17.9% of the total PWH population in the EMA and significant unmet need to access HIV primary care services. Black non-Hispanics are disproportionately affected by HIV. Blacks were 39.8% of new cases and 39.2% total PWH but only 15.9% of the general population. Hispanics are 41.3% of new HIV cases but only 31.6% of the general population. The Department of Health in Orange County (DOH-Orange) Sunshine Care Center service area (Orange, Osceola, & Lake counties) represents 90% of the HIV infection cases (PWH population) in the Orlando EMA. The service area has the largest HIV+ population & Black non-Hispanic PWHs (Orange County), includes rural areas (Lake & Osceola Counties), and the majority of Hispanic HIV+ clients (Osceola County). Key Services to be supported: DOH-Orange has served clients infected with HIV since 1989 and has developed a comprehensive HIV treatment program as part of a larger public health service center. Outpatient HIV care services include: counseling and testing, test & treat, eligibility determination, primary and specialty medical care, mental health counseling, substance abuse care, nutritional counseling, screenings, adherence counseling, ADAP, health education & risk reduction, access to research, case management, support & advocacy services including peer mentoring; and facilitated referrals to oral health care, housing assistance, financial assistance, transportation assistance and other supportive care. DOH-Orange provided HIV primary care services to 1,174 unduplicated clients in 2021, 1,134 in 2022, and 1,083 in 2023. Summary Request: DOH-Orange is requesting Ryan White Part C EIS funding in the amount of $1,074,279 per project year to continue serving the entire service area of: Orange, Osceola, and Lake Counties Florida. DOH-Orange provides HIV primary care in an outpatient setting through a comprehensive client-centered continuum of HIV primary care services. Funding preference under Qualification 1 Increased Burden; Qualification 2: Rural Areas; and Qualification 3: Underserved Areas is requested. Target Populations: Our target population has been and continues to be African American, Hispanic ethnicities, women, youth, as well as homeless and incarcerated persons who are infected with HIV. By the end of the 2025-2028 period, Part C funds will serve 945 unduplicated clients with 180 new clients. Client demographics: 54% African American, 30% Hispanic, 68% male, 31% female, 1% transgender; and 52% will be uninsured/underinsured. Objectives for HIV patients served: By 2028, 95% of patients will have a viral load<200copies/mL and 98% will be prescribed ART.
