OP EARLY INTERVENTION SERVICES WITH RESPECT TO HIV DISEASE - Project Abstract People living with HIV (PLHIV) across Vermont rely on the University of Vermont Medical Center’s (UVMMC) Ryan White Comprehensive Care Clinics (CCC) for their healthcare. This model of care includes one major HIV clinic in the state’s largest county and four satellite clinics providing care across the state’s four quadrants and central Vermont. The CCC, over its 30 years of experience, demonstrates profound viral suppression (93%) and meaningful engagement into care. The model also strives to embrace a patient-centered care approach with an innovative HIV care model that enables PLHIV a voice and positive community across Vermont. Comprehensive HIV care across the UVMMC CCC sites includes dedicated outreach and partnership with relevant community allies to enroll patients into care in a timely manner and many means to keep individuals engaged into care. HIV clinical needs that need to be addressed across the catchment area for UVMMC include serving as the primary HIV care program for the state with some patients accessing care from areas just beyond its borders. UVMMC CCC serves as Vermont’s primary HIV care provider with comprehensive services to enhance patient well-being. The UVMMC provides care for between 515-560 individuals across the 2022 and 2023 timeframe which encompasses the majority of PLHIV in the state. Services by the UVMMC CCC include evaluating and caring for PLHIV with attention paid to all immune and infection associated markers that accompany HIV, prescribing anti-retroviral therapy (while utilizing Vermont’s AIDS Drug Assistance Program, when needed), ensuring risk reduction counseling, delivering primary care, providing referral for dental and eye care, and coordinating HIV peer support and community engagement. Populations served by the UVMMC CCC include a substantial proportion (78%) of individuals 45 years or older, mostly (75.9%) white and largely (84%) comprised of cisgender men. Despite Vermont’s racial mix revealing profound homogeneity with 92% of the population identifying as white, the CCC’s demographics include 12.7% of individuals enrolled into care that self-identify as Black/African American. The UVMMC CCC provides a unique care delivery model that includes telehealth engagements that preceded the Covid-19 pandemic and satellite rural clinic sites to reduce transportation barriers for patients living with HIV across rugged and rural landscapes.
