Overview of HIV Epidemiology in Service Area Miami-Dade County, the site of our University of Miami Miller School of Medicine (UMMSM) Ryan White Part C program, continues to report the highest rates of HIV and AIDS in the country and is among the 48 counties being targeted to promote the strategies of the national Ending the HIV Epidemic Initiative. In 2021, it ranked first in the rate of new diagnoses among U.S. Eligible Metropolitan Areas at 35.2/100,000, a rate more than three times the national rate of 10.8. It ranks second in prevalence rates, with 1,020.3/100,000, while the U.S. rate is 324.6. The number of people living with HIV (PWH) in Miami-Dade County in 2022 totaled 28,479, with 1,088 new HIV diagnoses being reported. The incidence of new HIV cases in Miami Dade increased by 52% since 2020 (from 704 to 1,088), with the majority being among males (84%); females accounted for 15% and transgender women one percent. The Florida Department of Health estimates that 14% of patients are unaware of their HIV status in the state and reports that 7,823 (27%) PWH in Miami-Dade County did not have a documented viral load or CD4 lab, medical visit or prescription between January 1, 2022 through June 30, 2023. B. Description of Key Services to be Support The UMMSM Part C program is a partnership between the Departments of Medicine’s Adult HIV Service and Obstetrics, Gynecology and Reproductive Sciences Women’s HIV Service, which ensures that female patients have on-site access to gynecological and prenatal care. Based at an academic medical center, patients have access to primary HIV care with board certified internists and/or infectious disease providers as well as to numerous other on-site HIV specialty clinics, established to enhance care for individuals with co-infections and other complex conditions. Nurse practitioners and a physician assistant provide primary care as well as gynecology services, and a psychologist provides on-site mental health assessments and treatment or referral for on-site psychiatric services as needed. Ancillary staff support the clinic structure and provide additional supportive services and education to assist patients with barriers that prevent their retention in care and achieving or maintaining viral suppression. C. Population to be Served The UMMSM RW Part C target population includes adults with HIV who live throughout Miami-Dade County. As the public hospital, our RW Part C clinics at UMMSM/Jackson Memorial Hospital (JMH) serve as the county catchment clinics for indigent, minority PWH. The zip code where our clinics are located has one of the highest concentrations of newly diagnosed HIV cases in the county, with a rate of 35.2/10,000 population, a rate eight times higher than the countywide rate (4.4). Over 95% identify as a racial or ethnic minority, and almost half are foreign born. Part C serves a large middle-aged and elderly population; 70% are over age 45 and have been diagnosed with one or more co-morbidities which complicates their care Almost 62% of patients ages 45-59 and 73% of those age 60 and older have an AIDS diagnosis.
