Chase Brexton is the current grantee for the service area and operates the following centers within the catchment area: Mt. Vernon Center in Baltimore City, Glen Burnie Center in Anne Arundel County; Security Square Center in Baltimore County; and the Columbia Center in Howard County. Per the 2022 Baltimore Metropolitan Region Annual Epidemiological Profile published by the Maryland Department of Health, there are an estimated 18,198 persons living with HIV throughout the Baltimore MSA. Among these persons, 91.8% are diagnosed, 75.7% are retained in care, and 62.2% are virally suppressed. Founded in 1978, Chase Brexton began providing HIV primary medical care and support services to persons with HIV (PWH) in 1988. Chase Brexton has been the recipient of Part C funds since 1991, and the grantee of record since 1995. This project will allow Chase Brexton to continue to provide high-quality, patient-centered HIV primary medical care and support to low-income and often under-served populations living with, and affected by, HIV throughout the service area. Per Chase Brexton’s 2023 Ryan White Services Report, our patient population with HIV often have numerous physical and psychological comorbidities, including untreated behavioral health and substance use disorders. They also face a variety of socioeconomic barriers to care, including poverty, lack of third-party payment, lack of transportation, poor health literacy, and housing and food insecurities. This project will continue to reduce barriers to access and retention in care among the target populations, including improving access to antiretroviral therapy for PWH and HIV prevention services for populations impacted by HIV-related health disparities. As a Certified Patient-Centered Medical Home, Chase Brexton provides a variety of medical and psychosocial support services that are fully integrated across the HIV Care Continuum, including: HIV primary medical care; HIV testing and diagnostic services; Community outreach and linkage to care; LGBTQ- and Gender-affirming healthcare; Medication Management Therapy and free medication delivery; Behavioral Healthcare; and Oral Health. The key services supported by this request include: HIV primary medical care; HIV testing and linkage to care services; Health Education and Risk Reduction Counseling; and Medical case management. The populations of focus for this project include low-income PWH throughout the service area, as well as populations disproportionately impacted by HIV-related health disparities, including: African American same-gender loving men; Transgender women; Injection drug users; African American heterosexual cis-women; Hispanic/Latinx LGBTQ-identifying persons; and LGBTQ-identifying adolescents and young adults.
