BronxCare draws patients from communities throughout Bronx (the entire service area designated in the NOFO Appendix C), with a primary service area encompassing the following United Hospital Fund Neighborhoods (designated by south and central Bronx zip codes): Highbridge-Morrisania (zip codes 10451, 10452, and 10456); Hunts Point-Mott Haven (10454, 10455, 10459, and 10474); Crotona-Tremont (10453, 10457, and 10460); and parts of Fordham-Bronx Park (10458 and 10468). The primary service area neighborhoods are designated as HRSA Health Professional Shortage and Medically Underserved Areas with predominantly minority populations experiencing high poverty, limited educational attainment, and a large proportion of community residents who are uninsured. BronxCare serves communities that are among the poorest in the nation. These socioeconomic factors synergistically compound existing barriers to health care access and treatment. The Bronx, as well as the south and central Bronx, are disproportionately affected by HIV/AIDS, in comparison to the NYC Eligible Metropolitan Area (EMA). Of the 132,003 PLWHA in NYC as of 2022, 24% (or 31,254 persons) resided in the Bronx where the HIV incidence rate (25.5 per 100,000 persons) was higher than the NYC EMA (19.2 per 100,000 persons). Data from the BronxCare PSA neighborhoods reveals higher HIV incidences?Hunts Point-Mott Haven (35.3 per 100,000), Crotona-Tremont (34.5 per 100,000), and High Bridge-Morrisania (33.5 per 100,000)?and prevalence rates (2.9%, 3.0%, 3.0% respectively) compared to both the Bronx (2.2%) and the NYC EMA (1.6%). In the PSA neighborhoods, HIV/AIDS prevalence was highest in High Bridge-Morrisania (6,451 PLWH), yet HIV incidence was highest in Hunts Points-Mott Haven (35.3 per 100,000 persons). BronxCare's AIDS Program provides comprehensive, integrated, outpatient primary care services in accordance with HRSA Ryan White Program guidelines. The Part C grant program funding request will enable the AIDS Program to continue the provision of core medical services (outpatient and ambulatory health services, medical case management including treatment adherence, medical nutrition therapy, outpatient mental health screening and treatment, and substance abuse outpatient care) and support services (health education/risk reduction support groups and medical transportation; with client referrals to service area community-based organizations that will provide additional supportive services). The AIDS Program will continue a longstanding dedication to subpopulations most highly impacted by HIV/AIDS including: immigrants from West Africa, as well as Central and South American countries, and young men who have sex with men of color (ages 18-35 years). These subpopulations require targeted HIV care continuum interventions with outreach and testing to achieve HIV viral suppression.
