(formerly Hudson River HealthCare), a New York State licensed, federally qualified health center (FQHC) recognized by the National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Home, is seeking support to maintain and strengthen its successful Genesis program of HIV care, treatment, and supportive services in the Hudson Valley and New York City. • Needs to be addressed – Sun River Health is proposing to serve its existing Ryan White Part C EIS service area which encompasses the three NY State Hudson Valley counties of Westchester, Dutchess, and Sullivan as well as high-need neighborhoods in Bronx (zip codes: 10451, 10452, 10454, 10455, 10456, 10459, 10474), Kings (zip codes:11201, 11203, 11205, 11213, 11216, 11217, 11225, 11238), and Queens (zip codes: 11412, 11423, 11432, 11433, 11434, 11435,11436) counties in New York City. The New York State HIV/AIDS Surveillance Annual Report for Cases Diagnosed through December 2022 (most recent report available) reveled that, in 2022, there were 2,860 individuals with HIV in Westchester, 704 in Dutchess, 234 in Sullivan, 23,602 in Bronx, 21,757 in Kings (Brooklyn), and 13,455 in Queens. In addition, the New York State Department of Health HIV Cascade of Care data for 2022 (most recent year available) estimates that, of those individuals with diagnosed HIV, only 87% in the Mid-Hudson, 88% in the Lower Hudson, and 88% in New York City Ryan White Regions are receiving HIV care; this means that there are 11,240 persons with HIV across these regions who are not engaged in treatment. • Proposed services – Sun River Health is requesting $974,122 to continue meeting the needs and addressing the gaps in HIV care delivery in the Hudson Valley and New York City. Genesis is a comprehensive, integrated, multi-disciplinary model of HIV care and treatment designed to meet the complex needs of the Health Center’s patients with HIV. It provides high-quality, culturally and linguistically competent primary, preventive, behavioral and oral health care, screening and treatment for substance use disorder, specialty care, as well as a full range of support services and seamless linkage to a highly responsive referral care network, as needed, for those with HIV. The Health Center will use Part C funding to support core medical and support services including outpatient/ambulatory health services; medical case management, including treatment adherence; health education/risk reduction services; and outreach services. • Population groups served – The Health Center’s Genesis HIV treatment program targets individuals with HIV from low-income and medically underserved communities with a particular focus on subpopulations who are disproportionately infected with, and affected by, HIV including African Americans, Hispanics/Latinos, men who have sex with men (MSM), women, people who inject drugs, and individuals experiencing homelessness.
