The purpose of the NYIC 4-in-1 program is to ensure that comprehensive, culturally acceptable personal and public health services are available and accessible to NYC area urban Indians and to promote the highest possible health status for NYC urban Indians by providing services in each of the following 4 areas: (1) Health Promotion/Disease Prevention services, (2) Immunization Promotion services, (3) Alcohol and Substance Abuse-related Promotion services, and (4) Mental Health services. NYIC 4-in-1 program services implement evidence-based and culturally-based activities to address the following health priorities. The health priorities are based on the identified local needs in this proposal, identified service/infrastructure gaps, and Area Tribal health priorities: 1. Opioid Abuse 2. Substance Abuse Rehabilitation and Aftercare 3. Hepatitis C 4. Obesity 5. Health insurance enrollment 6. Diabetes prevention, including gestational diabetes prevention 7. Cardiovascular disease 8. Cancer 9. Influenza and respiratory illness The New York Indian Council (NYIC) is a nonprofit subsidiary organization of the Rhode Island Indian Council (RIIC), Inc., an American Indian-governed 501(c)(3) nonprofit organization. Since 2012, when NYIC was awarded the U.S. Department of Labor (DOL) Workforce Investment & Opportunities Act funds to serve urban Indians in New York City, NYIC proudly serves an annual, growing clientele of over 400 New York City area urban Indians of federally recognized Tribes. In October 2018, the New York Indian Council became a Title V Urban Indian Health Program (UIHP), funded by Indian Health Services, and began providing health services to the urban Indian community of the NYC metro service area. NYIC provides its health and wellness services through its NYIC “Indian Health Services” Department and its employment services through its NYIC “WIOA” Department. All clients provide their Tribal identification and proof of residence as required, in addition to other supportive documentation.
