First Nations Community HealthSource, a Title V Urban Indian Health Center in Albuquerque, New Mexico will conduct the 4 in 1 Project to address the significant unmet health, behavioral health, and HIV prevention needs among Albuquerque's urban Indian community. Funds will also be used to increase FNCH's capacity to implement, sustain and improve effective health care services to urban Indians. HIV prevention and immunization activities will target a minimum of 100 American Indian/Alaskan Natives each. Substance abuse and mental health services will target a minimum of 50 American Indian/Alaskan Natives each. Evidence-based practice models will be incorporated into services and a program evaluation will be conducted to assess the effectiveness of the 4 in 1 services and to continuously improve program services. The proposed services will address gaps and barriers in the existing service system and resources will be leveraged to increase the center’s capacity to conduct 4 in 1 activities.
