The Connecticut Early Hearing Detection and Intervention Program (CT EHDI) proposes to improve infrastructure to ensure ALL Connecticut-born newborns are screened by 1 month of age, diagnosed by 3 months of age, enrolled in EI by 6 months of age (1-3-6), lost to follow-up/lost to documentation is reduced, and improve disparities to optimize development. CT EHDI will also support hearing screening in young children up to age 3, continue to develop relationships with community partners, and develop new relationships with the three national technical centers. The program will also identify language acquisition data sources and develop infrastructure to capture and analyze data to address disparities. Family engagement remains a priority and CT EHDI will continue its contract with its family-based organization (FBO) to support and engage families with DHH children. In concert with our FBO, CT EHDI will also engage, educate, and train health professionals and service providers on 1-3-6; the need for hearing screening up to age 3; the family-centered medical home; and the importance of communicating information to families. NEEDS TO BE ADDRESSED: 1. Maintain or improve screened by one month of age rate. 2. Increase the rate of diagnostic audiologic exams completed by three months of age. 3. Reduce delays in enrollment into Birth to Three (B23) by six months of age. 4. Reduce Lost to Follow-up\Documentation associated with 1-3-6. 5. Identify a partner(s) that can provide data sources for “language acquisition”, referred to herein as “LA”. 6. Develop an MOU(s) or family waiver(s) with LA partner(s) and legal departments. 7. Determine standards, baselines, and ben chmarks for LA. 8. Develop a method of data sharing, tracking, and reporting LA. 9. Identify, develop, or improve associated required software\hardware infrastructure for tracking and reporting LA. 10. Improve physician knowledge of the EHDI system, 1-3-6, Medical Homes, screen through three, and EHDI best practices. 11. Support hearing screening in young children up to age 3. 12. Continue supporting DHH family engagement through our contracted Family Based Organization (FBO). THE PROPOSED SERVICES: 1) By the end of year one, CT EHDI will collect and report the following baseline data: a) Strengthening the infrastructure: i) The number of partnerships built with other EHDI stakeholder organizations and entities who can provide LA data, collaboration, and support to improve language acquisition outcomes. b) Providing family-to-family supports and services: i) Continue to provide family-to-family supports, engagement, education, and services, including DHH adult-to-family ambassadorship. 2) By end of Year 5, CT EHDI will collect and report baseline LA outcomes for the population of 3-year-old DHH children. 3) By March of 2029, using the 2021 CDC HSFS 1-3-6 data as a baseline, CT EHDI will: a) Achieve at least a 98 percent screening rate of the number of infants that completed a newborn hearing screen no later than 1 month of age. b) Increase by 10 percentage points from baseline the number of infants that completed a diagnostic audiological evaluation no later than 3 months of age. c) Increase by 15 percentage points from baseline the number of infants identified as DHH that are enrolled in EI services no later than 6 months of age. THE POPULATION TO BE SERVED: All Connecticut-born children, up to three years old, including, but not limited to: children who have incomplete or non-passing hearing screenings; children with an incomplete or no diagnostic hearing exam; children identified as DHH; children not enrolled in Early Intervention services; and the fam ilies of DHH children
