The problem of infant mortality and perinatal health disparities is seen throughout the county. There are several cities that make up Jefferson County with Birmingham being the largest. However, BHSP will continue to target the cities of Birmingham, Bessemer, and Fairfield, adding Center Point as the designated areas to employ the strategies and activities aimed at improving perinatal outcomes. All cities are located within Jefferson County, Alabama. The BHSP program continues its work towards improving health outcomes before, during, and after pregnancy for African American women (the primary target population), and other women, residing in the target area; and towards reducing infant deaths and adverse perinatal outcomes among women giving birth in the program target area of Jefferson County, and reduce the racial/ethnic differences in rates of death and adverse perinatal outcomes. BHSP will continue to work collaboratively with local community-based organizations, local, and statewide MHC, and other medical, behavioral health and social services partners to address the SODHs faced by AA pregnant, non-pregnant women and their families. The three-year (2019-2021) infant mortality rate (IMR) for the area is 11.2/1,000 live birth, with 13.7 for the Targeted population (AA women), significantly higher than the program eligibility threshold of 8.2/1,000. BHSP will provide Direct and Enabling Services to 700 unduplicated BHSP program participants (at least 450 through case management/care coordination, and 250 unduplicated participants through group-based health and parent education), and other program specific and collaborative services to address the SDOHs that negatively impact our communities and lead to poor perinatal outcomes for women among our targeted population.
