Address: 439 Port Richmond Avenue, Staten Island, New York 10302 Project Director: Dr. Henry Thompson, DSc, FACHE, Chief Executive Officer Phone: (718) 924-2254 Fax: (917) 830-1976 E-Mail: hthompson@chcrichmond.org Web: www.chcrichmond.org Funding Requested: Federal Healthy Start funding of $1,100,000 per year for 5 years Background: Community Health Center of Richmond (CHCR) is a Federally Qualified Health Center (FQHC), and has been an existing Healthy Start (HS) grantee since 2014. CHCR operates 4 service delivery sites located in Staten Island (Richmond County), New York. CHCR served 11,256 unique individuals with 35,237 visits in calendar year 2022. Services provided by CHCR include integrated primary medical (including OB/GYN and pediatrics), dental, behavioral health, nutrition, WIC, some specialty care, and enabling services. HS Services: With Federal funding to support our existing HS Program, we will continue to offer a comprehensive array of services for HS participants, including: case management; nutrition counseling; community outreach for enrollment in Medicaid and other health insurance; breastfeeding support; doula services; health education for safe sleep practices, breastfeeding, immunizations, birth spacing, and parenting; group prenatal care (as of January 2024); and linkage to supportive services throughout the community. Last year, CHCR’s HS Program enrolled 702 new participants and served a total of 1,331 program participants. Project Description/Purpose: The Staten Island HS Program aims to improve health outcomes before, during, and after pregnancy, as well as to reduce racial/ethnic disparities in rates of infant death and adverse perinatal outcomes. Through our HS project, existing services will be expanded, including the addition of group prenatal care based on t
he evidence-based Centering Pregnancy model. CHCR is requesting continued HS funding to improve perinatal health outcomes, promote the health of women, infants, and families, and reduce racial/ethnic disparities in perinatal outcomes, including rates of infant death. Service Area: The geographic service area for this project is Richmond County (known as Staten Island), New York. The defined geographic boundaries of the service area include all of Staten Island, which is an urban area that is one of the 5 boroughs of New York City. The service area includes multiple federally designated Health Professional Shortage Areas (HPSAs) for medical, dental, and mental health services within the community. A portion of the service area is also federally designated as a Medically Underserved Area (MUA). Target Population/Needs: The target population for the Staten Island HS Program is Black non-Hispanic women, as well as their partners/fathers and children, within the geographic service area of Staten Island. This target population was selected because they experience the highest rates of infant mortality in the service area, due in part to stark racial disparities for Black women and infants in our community. In addition, social determinants of health pose barriers to accessing care for this target population. Summary: CHCR has a lengthy history of collaborating and coordinating with other agencies on Staten Island to document both quantitative and qualitative positive impacts on maternal, infant, and child health. CHCR has a strong leadership infrastructure, including an experienced management team and an engaged Board of Directors. CHCR is a sophisticated organization that offers a comprehensive, integrated scope of services, supported by QI, EHR, clinical, financial, and operational systems that position us to successfully continue the implementation of our Healthy Start program.
