Address: 231 East Jefferson Street, Quincy, FL 32351 Project Director Name: Sharon Ross-Donaldson, LCSW Contact: VOICE (850) 875-5005 FAX (850) 875-4945 Email: srdonaldson@centerforhealthequity.com Web: www.centerforhealthequity.com Request: Year 1 $1,100,000 and Year 2-5 $1,100,000 per year for Healthy Start Initiative: Eliminating Disparities in Perinatal Health PROJECT DESCRIPTION Gadsden County, an impoverished rural community, has one of the highest infant mortality rates, low birth weight, and other poor birth outcomes among African Americans in Florida. The Gadsden Federal Healthy Start Project (known as Gadsden Woman to Woman) provides outreach, education, case management/care coordination, and behavioral health services (where appropriate) to African American pregnant and pre- and interconceptional women who are at risk for a poor birth and health outcomes as well as their infants/children and father/male partners. The Project works collaboratively with other service sectors/providers to integrate, expand, and strengthen the maternal and child health system of care in Gadsden County and eliminate disparities in perinatal health. The overarching goals for GFHSP are to reduce infant mortality and decrease disparities in infant mortality and poor perinatal health outcomes in Gadsden. GFHSP will utilize HRSA funding to serve a rural project area.
