Project Abstract Project Title: Central Oklahoma Healthy Start Initiative Applicant Organization: Community Health Centers, Inc. Address: P.O. Box 30589, Oklahoma City, OK 73140-3589 Project Director: Tahirah Watley Contact Numbers: Ph: (405) 427-3200 X 3227; Fax: (405) 427-3212 Email Address: twatley@communityhealthok.org Website: https://communityhealthok.org/healthy-start-initiative-okc/ Funds Requested: Year 1-5: $1,100,000 each year Funding Priority: Priority 1 (Oklahoma County) and Priority 2 (Pottawatomie County) Service Area: Oklahoma County- Urban; Pottawatomie County- Rural Community Health Centers, Inc., (CHCI) is a community-based 501(c)(3) private non-profit corporation providing over 50 years of uninterrupted health care services, primarily to low-income uninsured and underinsured Oklahomans, who often experience difficulties in accessing culturally and linguistically sensitive care. CHCI offers preventive and primary health care, behavioral health and oral health care services at seven (7) sites, including a homeless health center in Oklahoma City. As a current Healthy Start Initiative: Eliminating Disparities in Perinatal Health grantee, CHCI is submitting a Health Resources and Services Administration (HRSA) Competing Continuation Grant for Funding Opportunity Number: HRSA-24-033. CHCI has been a recipient of Healthy Start Initiative funding since 2001 and sponsors the Central Oklahoma Healthy Start Initiative (COHSI) program with a motto of “Healthy Babies Begin Before Birth” (H4B). The COHSI program is aimed at improving health outcomes before, during, and after pregnancy, and reducing racial/ethnic differences in rates of infant deaths, adverse perinatal outcomes, and maternal morbidity. Priority 1: COHSI will serve to reduce the disparity in the health status between the general population and racial/ethnic minority groups, with a focus on the Black/African American infants and families with a docume nted 76 deaths for an infant mortality rate (IMR) of 13.1/1,000 population in urban Oklahoma County from 2019-2021. Priority 2: For the 2019-2021 reporting period, rural Pottawatomie County qualifies for this priority with 15.3% of Pre-pregnancy gestational hypertension; 38.5% Pre-pregnancy obesity; and 68.6% Entering prenatal care in the 1st trimester. The COHSI program has improved access to quality health care and services for women, infants, children and families by providing outreach, case management, care coordination and health education while providing linkages to health insurance coverage. The COHSI workforce is strengthened by being located at Perry A. Klaassen Family Medical Center, one of CHCI’s federally qualified health centers (FQHC), where a licensed Family Practice physician with OB Track and support staff are available onsite as well as all COHSI staff. This greatly reduces the barrier of transportation and keeps all services coordinated at one location for an effective delivery of health care and will continue to improve participation in the COHSI program. COHSI is committed to strengthening and reaching all MCHB Goals 1-4 by ensuring access to high quality and equitable health services for all populations. CHCI is committed to serving no less than 700 unduplicated participants annually with at least 450 participants through case management/care coordination and a minimum of 250 participants through group-based health and parenting education. Case management/case coordination will be provided for at least 450 participants annually with a minimum of 250 pregnant women and 25 fathers/partners. The remaining participants will be pregnant women; preconception women; interconception women of reproductive age; infants/children from birth to 18 months; and fathers/partners with a pregnant partner or who have an infant younger than 18 months of age and/or are the current or former partner of an enrolled participant.
