The Center for Black Women’s Wellness (CBWW) is a community-based, non-profit organization committed to improving the health and well-being of underserved Black women and their families. In response to Funding Opportunity Number HRSA-24-033, CBWW is applying for Healthy Start funding to continue its critical work of improving maternal child health outcomes among perinatal Black women in Fulton County, Georgia, and urban area. In Fulton County, non-Hispanic Blacks have consistently experienced higher rates of infant mortality than their non-Hispanic White counterparts. The average Infant Mortality Rate (IMR) in Fulton County between 2019 and 2021 was 7.7, with a Black rate of 10.8 and a White rate of 3.8. In addition, while every infant death is a tragedy, between 2019 and 2021, there were 226 infant deaths in Fulton County, with a total of 182 Black infant deaths meaning that Black infants comprised 81% of total infant deaths during that period. Atlanta Healthy Start will build upon its strides made in addressing poor birth outcomes and provide culturally and linguistically direct and enabling services to a total of 700 individuals annually. The project will leverage partnerships, curricula, and evidence- based/evidence-informed and/or scientific information to implement high quality case management services and group health education services. AHSI will utilize a mixed provider model to provide case management services and execute a variety of engaging health promotion activities. Program elements will also include mental health services, fatherhood services, doula linkage, and women’s health and preventive health services. The project will leverage its built community leadership network to expand its community consortium with capacity to guide activities and drive meaningful change in maternal child health outcomes. An evaluation plan will support the tracking of all p rogram goals and objectives.
