The Great Plains Tribal Leaders Health Board (GPTLHB) is a community-based 501(c)(3) tribal entity that provides culturally credible approaches to public health support and healthcare advocacy to the 17 tribal nations and one service area in the Great Plains area (GPA) of South Dakota, North Dakota, Nebraska, and Iowa. Founded in 1986, GPTLHB stands as a formal representative board designed to advocate for tribal health concerns and work with tribal communities to improve health status and eradicate health disparities among the region's estimated 265,837 tribal members. GPTLHB’s mission is to provide quality public health support and healthcare advocacy to the tribal nations of the Great Plains by utilizing effective and culturally credible approaches. Our vision is that all Tribal nations and communities in the GPA will reach optimum health and wellness through lasting partnerships and culturally significant values empowered by tribal sovereignty. “Hecel Oyate Kin Nipi Kte,” so that the people may live. As a current HRSA Healthy Start Initiative awardee, GPTLHB recognizes the imperative continued need to improve health outcomes before, during, and after pregnancy and to reduce the racial and ethnic differences adversely impacting rates of infant death and perinatal outcomes for AI/AN in the region. Due to small number cell suppression and longstanding challenges in receiving data from state and federal databases, data is currently only available for AI/AN living in ND and SD. From 2019 through 2021, there were 7,344 births to AI/AN women in the two states (ND: 2,445; SD: 4,899), with a total of 86 deaths (ND: 27; SD: 59) and an infant mortality rate of 11.71 per 1,000 (ND: 11.04; SD: 12.04). 2019-2021 data on preterm and low birthweight births is available only for SD counties that include AI/AN reservations, where 655 AI/AN infants were born prematurely, and 400 had a low birth weight for rates of 13.6/100 and 8.3/100, respectively. To e fficiently and accurately ensure the delivery of services to at least 50% of this target population, GPHS clinical and educational services will be provided virtually, in person, and/or via a referral system in locations where services do not currently exist. The proposed Great Plains Healthy Start (GPHS) program’s target population will include AI/AN residing in the GPA - in a project geographic area encompassing 311 rural and urban counties across the four-state region - who experience high or above the national average annual infant mortality rates. GPHS will provide direct and enabling services by hiring two Care Coordinators to provide family-centered, strengths-based, evidence-based, and culturally appropriate virtual case management/care coordination to GPHS participants. A Health Educator will facilitate in-person and virtual group parenting educational sessions for participants. In coordination with a contracted telehealth organization, GPHS will develop a mental health assistance helpline line to encourage the anonymous counseling of mothers with substance use disorder (SUD) and other behavioral health habits that can negatively impact pregnancy. The program will also convene diverse, multi-sector state, local, and community level Consortia to advise GPHS activities, address disparities in perinatal health outcomes, and implement plans to improve the quality of and access to clinical, support, and education services within the project geographic area.
