Emphasizing direct services like screening, referrals, and health education, the program also stresses the importance of community consortia to develop strategies and interventions tailored to local needs. With goals to reduce infant mortality rates and decrease disparities in maternal and infant health outcomes, Family Solutions will implement cross-sector partnerships and community-based resources, emphasizing social determinants like housing, transportation, and food access. The project will span five years, aiming for significant improvements in perinatal health outcomes in rural SC. Family Solutions, with a 26-year legacy of dedicated service to the priority population, is uniquely positioned to drive the Healthy Start Initiative.
