The current grant award number for the Des Moines Healthy Start Project is: H49MC00052. Under the HRSA-24-033 funding opportunity, EveryStep will continue the provision of comprehensive maternal, child, family and community development services for preconception women, pregnant women, interconception women, children birth to 18 months and fathers/partners as performed by the current Des Moines Healthy Start Project (DMHSP) in an expanded target area. The DMHSP will provide and increase efforts to improve health and wellness for women, children and families; promote quality services to improve maternal, birth and infant outcomes; decrease racial disparities in maternal and infant mortality rates in addition to poor birth outcomes; strengthen family resilience; and achieve collective impact through the DMHSP consortium. The project will also continue to develop community collaboration, participate in a shared measurement approach, coordinate resources, and conduct performance evaluation. All DMHSP services under the HRSA-24-033 funding opportunity will be provided to preconception women, pregnant women, interconception women, infants/children up to 18 months, and fathers/partners with a partner who is pregnant or has a child under 18 months living in nine Z
IP Codes in Des Moines and its metropolitan area with high rates of disparity in infant mortality (IM) and poor perinatal health outcomes (50009, 50265, 50266, 50312, 50313, 50314, 50316, 50317, 50320), an urban project area. The purpose of the DMHSP is to continue to address significant disparities in perinatal and infant health indicators. Vulnerable marginalized communities comprise the target population for the DMHSP; and the Black/Asian/Other populations will be specifically targeted for program services. Efforts to enhance the current community’s perinatal service system will continue through the utilization of community-driven strategies that address the needs of preconception, high-risk pregnant and interconceptional women, their infants/toddlers, and their child’s father or participant’s partner. DMHSP is the only Healthy Start project in the state of Iowa.
