The Florida Office of Rural Health (FORH) is applying for the Small Rural Hospital Improvement Program (SHIP) funds for 14 eligible small hospitals in rural Florida communities. Each SHIP hospital will work on an annual project which is consistent with the five-year goals of continuing to move their facilities forward with federal and state healthcare reforms transitioning them to a value-based care delivery setting. All SHIP eligible facilities are actively strengthening their ability to access and report quality and value, to support accountable care organization and shared savings principles, as well as improve their financial processes. Each year of the grant, each hospital’s annual project will be determined via dialogue with the SHIP eligible hospitals and the FORH staff. Florida’s efforts to specifically improve rural health are supported primarily by funding from three federal grant programs, the State Office of Rural Health program, the Medicare Rural Hospital Flexibility Program and the Small Rural Hospital Improvement Program. Although the focus and activities of each grant program differs, the activities of these grants have been integrated by FORH into a comprehensive strategy to address Florida’s rural health goals.
