Needs to be Addressed The goal of this program is in keeping with the mission of the Tennessee Department of Health which is to protect, promote and improve the health and prosperity of the people of Tennessee. The mission, as it applies to the unique health needs of the Tennessee rural population is aligned with the Tennessee Health Access Act of 1989 which exists in response to a predominate need for enhancing access to health care. Small rural hospitals serve as the hub of the health care infrastructure in rural communities providing services to local rural residents in need of convenient quality health care. The impact on the local community is better access to care and thereby a reduction in rural health disparities. Proposed Services The Tennessee Department of Health State Office of Rural Health is committed to providing a Small Rural Hospital Improvement Program for small hospitals with 49 beds or less to embrace the challenges of achieving value based care goals that improve rural health outcomes. To that end, the Tennessee State Office of Rural Health will provide technical support and assist eligible hospitals with investing SHIP funding to attain: value-based care goals, join or become accountable care organizations or create shared savings programs; utilize health information technology software or efficiency training in support of the aforementioned programs, in addition, to achieving compliance with ICD-11 standards and payment bundling. Further, the Tennessee State Office of Rural Health will provide consultative support and monitor measurement strategies to gauge the level of successful achievement of desired SHIP investment outcomes for participating hospitals. Population Group(s) to be served The State Office of Rural Health purposes to focus SHIP initiatives that alleviate access to care issues among the chronically ill, aging, uninsured, low-income, Medicaid (TennCare) and Medicare rural residents in Tennessee. Small rural community hospitals and Critical Access Hospitals are typically the local access point available to support the unique health care needs of their respective rural patient population.
