The South Dakota Small Rural Hospital Improvement Program (SHIP) team is a stakeholder group and together addresses the most pressing needs of the state’s small rural hospitals to ensure quality of care, patient safety, and viability. The stakeholders consist of the Office of Rural Health, members of South Dakota’s small rural hospitals, systems members from Avera Health, Sanford Health, and Monument Health, South Dakota Association of Healthcare Organizations (SDAHO), and members of the South Dakota Association for Healthcare Quality (SDAHQ). The 2023-2028 South Dakota SHIP grant application focuses on support for small rural hospitals in Value-Based Purchasing (VBP) investment activities, Accountable Care Organization (ACO) or Shared Savings investment activities, and Payment Bundling (PB) or Prospective Payment System (PPS) investment activities. Areas supporting VBP activities include quality metric data collection and related training or software, Medicare Beneficiary Quality Improvement Project (MBQIP) data collection and training, efficiency or quality improvement training in support of VBP related activities, provider-based clinic quality measures education, and alternative Payment Model and Quality Payment Program training and education. Areas supporting ACO include activities relating to computerized provider order entry implementation or training, pharmacy services training, hardware/software, and machines, Population Health or disease registry training and/or software/hardware, Social Determinants of Health screening software/training; efficiency or quality improvement training in support of ACO or shared savings, systems performance training, mobile health and telehealth hardware/software, community paramedicine hardware/software and training, and health information technology training for value and ACOs including training, software, and risk assessments associated with cybersecurity. Payment Bundling supports activities such as ICD-11 software and training, efficiency or quality improvement training in support of PB or PPS related activities, S-10 cost reporting training, and pricing transparency training. The activities presented in this grant application were identified from the FY 2023 hospital applications and were then prioritized. The South Dakota SHIP Program is administered by 1 FTE by the Rural Hospital Program Coordinator and is part of the Office of Rural Health. The Assistant Administrator of the Office of Rural Health is the direct supervisor for the SHIP Program Coordinator.
