Community Health Aide Program-Tribal Assessment and Planning (CHAP-TAP) - IHC values the voice of the community and the team members who join the mission to serve the community. To that end, IHC proposes to continue to understand how best to overcome barriers related to clinical infrastructure, workforce barriers, training infrastructure and cultural inclusivity in services provided by prospective CHAP providers. To determine the feasibility, sustainability and level of readiness for implementation of a CHAP at the Indian Health Council, the follow goals have been strategically selected: (1) Refine and implement CHAP Orientation Materials, focus groups and interview guide to inform of systemic barriers that prohibit the complete integration of the CHAP into IHC’s existing health care system (2) Facilitate a 2024 Community Health Assessment (3) Assess the problems within the community related to behavioral, primary, and oral health via the 2024 CHA, IHC GPRA Report, 2023 CHAP TAP Outcome Report (3) Complete a cost analysis and program evaluation to propose clinic- and community-generated solutions to implementing a robust CHAP which will address problems within IHC’s vast service area.
