Shoshone-Paiute Tribes of the Duck Valley Indian Reservation Diabetes Program - The Shoshone-Paiute Tribes Diabetes Program has a well-coordinated plan to address type II diabetes. The community of Duck Valley has a prevalence rate of 14.6%. The Shoshone Paiute Tribes Diabetes Program (SPT Diabetes Program) will focus efforts on diabetes clinical care and management, and community prevention. The community-based prevention efforts, diabetes incidences and prevalence rates can be reduced and those living with type II diabetes can better manage the disease. With the strong support of leadership, medical teams' staff, and administration the Shoshone Paiute Tribes plans to address the best practice of foot exams and care. This will be done with the use of the up-to-date standards of care for foot care and with an integrated wholistic approach with coaching, care, and education at all levels of patient care. With case management efforts to identify and carry out the care required for best diabetes outcomes. The Shoshone-Paiute Tribes Diabetes Program will address prevention efforts with involvement from the community members. The Shoshone-Paiute Tribes is in the high desert ancestral territories of the Shoshone and Paiute Tribes and located 100 miles from emergency rooms, hospitals, and specialty care. The efforts of the SPT Diabetes program will greatly impact and improve access to otherwise inaccessible services and opportunities related to diabetes care, and prevention services. The Shoshone-Paiute Tribes and the SPT Diabetes Program work collaboratively to improve the health status of the community for current and future generations. The efforts will lean on the tradition knowledge, cultural practices, traditional foods, and traditional indigenous concepts to improve health outcomes in those at risk of type II diabetes and those currently diagnosed with type II diabetes. With the lack of much needed services, other barriers like access to a broad range of healthy foods is limited. With strong stakeholders like the Commodity Food Distribution Program, Summer Foods Program, Senior Citizens Programs, and the local Grocery Store. The Shoshone-Paiute Tribes Diabetes Program has been in existence in the community for 25 years. Within those years, with funding from the Special Diabetes Program for Indians the program has successfully expanded and improved diabetes care for the diabetic population. This has been done with a holistic, integrated care approach. This has been done with weekly diabetes clinics, diabetes education, registered dietitians, laboratory services, primary care medical visits, and referrals to fitness instructors. The relationships and trust built with the patient population is invaluable and the, collaborated with other tribal, and local partners to sponsor events for opportunities for fitness, education and awareness. The SPT Diabetes Program has crafted a fitness space in two locations for community members. One location is the Diabetes Wellness Center ideal for diabetes patients, and accessible, by wheelchair and physical therapy patients and is open from 8 AM -5 PM located at the Owyhee Community Health Facility. The other location is ideal for younger community members with access up until 9 PM, Monday -Friday located at the Human Development Center. The SPT Diabetes Program has established a well respected program in the community. The efforts of the Shoshone-Paiute Tribes Diabetes Program will be carried out with the support and collaboration of the diabetes patient care team and the stakeholders. The funding from the SDPI grant will fund three positions within the Owyhee Community Health Facility to carry out SDPI grant activities and build upon existing programs. The SPT Diabetes Program has the benefit of being in the Owyhee Community Health Facility where primary clinical care is provided. Efforts to address clinical care will continue and strengthened with the funds received from the SDPI grant.
