The Keweenaw Bay Indian Community (KBIC) Diabetes Program will provide diabetes treatment and/or prevention activities and/or services to Tribal Members living in our community based on our identified needs. We will implement SDPI Diabetes Best Practice (nutrition education) and report data on its Required Key Measure (Number and percent of individuals on the KBIC diabetes registry who are overweight who receive nutrition education. All activities/services will be aimed at reducing the risk of diabetes in at-risk individuals, providing high quality care to those with diagnosed diabetes, and/or reducing the complications of diabetes.
Based on our Diabetes Audit reports we would like to increase nutrition education, foot exams, physical activity education, and glycemic control to the diabetics in our Community. New collaborations will include health center representation on the local "Farmer's Market Coalition" which provides locally grown fresh produce that is less expensive than the grocery stores.
