Prevalence of diabetes and the impact of diabetes in AI/AN communities continues to be a threat. Additionally, diabetes is increasingly becoming a threat to all ages in AI/AN communities. This results in increasing access to medical and treatment care for diabetes, increase access to education and support services, increase access to support activities, and increase access to opportunities to make healthy lifestyle choices. This aims to develop and implement care that is comprehensive to account for the role that social determinants of health also play a key role to diabetes treatment and prevention. The significance of comprehensive diabetes treatment and prevention is that it informs all involved the understanding of social determinants of health by involving and collaborating with other support programs and entities.
