The Seldovia Village Tribe (SVT) owns and operates three rural clinics in Seldovia, Homer, and Anchor Point, Alaska, under the name SVT Health and Wellness. These clinics provide primary and preventative health care services for these communities and surrounding areas. We are located at the southern end of the Kenai Peninsula and are considered rural communities; Seldovia and Seldovia Village are rural and remote, accessed only by air or water. No roads connect these communities to the transportation hub in Homer.
SVT is a Tribal Health Organization in the Alaska area and serves the Seldovia Region. The Seldovia Village Tribe SDPI program is a part of the SVT Health and Wellness clinical programs that include primary, preventative, and dental services. The Alaska Native Medical Center (ANMC) in Anchorage is the statewide referral center and gatekeeper for specialty care. The Alaska Native Tribal Health Consortium (ANTHC) provides health promotion/disease prevention programs that are statewide in scope. SVT Health and Wellness patients receive specialty and referral care at ANMC and ANTHC.
SVT Health and Wellness strongly believe in empowering patients to learn as much as possible and utilize self-determination to best care for themselves. We believe that learning simple lifestyle changes can result in significant positive health outcomes, both natural and sustainable. SVT provides a clinic and community-based SDPI program and has chosen Diabetes-related Education as our Best practice in 2023. Additionally, SVTs full-time Health and Wellness Coach is available and accountable for connecting patients with individualized outreach, cooking classes, exercise classes, and other community connections to increase psychosocial support.
SVT requests $40,055 annually over a five-year project under the Indian Health Services - Special Diabetes Program for Indians (SDPI).
SVT Health and Wellness leadership, Tribal Council, our medical team of providers, and support staff are concerned with the health issues facing our AI/AN population with diabetes and the resulting complications resulting from diabetes. We are dedicated to monitoring and managing the care of patients with diabetes to improve the quality of life and the greater longevity of AI/AN patients.
