Consolidated Tribal Health Project, Inc. (CTHP) is requesting funds from Indian Health Service’s SDPI grant program to support its Diabetes Wellness Program, which has been in place since 1998. CTHP is located in Redwood Valley, California, and serves the counties American Indian/Alaska Native population. CTHP selected retinal eye screening as its best practice for the 2023 grant year, using GPRA performance data as part of its decision-making process. CTHP will improve GPRA performance indicators for retinal eye screenings by providing additional staff training and encouraging their participation in developing and implementing additional program activities to meet the need of our American Indian/Alaska Native diabetic patients. CTHP has a retinal camera on site, but the number of staff currently trained to use it has decreased with personnel changes. The Medical department manager and the Outreach manager will use this camera to train additional staff to use it that way there is always a trained staff person available to conduct the screening. Diabetics have a weakened immune system, taking extra precautions for this population is important because, with diabetes, the overall risk for catching the flu and having complications are far more than people who don’t have diabetes. The Community Outreach Services department oversees the Diabetes Wellness Program and provides patient outreach and community awareness activities. Outreach staff conduct home visits, participate in Tribal health fairs and cultural events, and provide education and support for patients to help them manage their health.
