Since at least 1989, Chapa-De Indian Health has provided culturally appropriate diabetes treatment and prevention services to American Indian/Alaska Native (AIAN) patients residing in Placer, Nevada, Sierra, and surrounding counties in California. Funding in the amount of $329,591 per year for five years is requested to fund the comprehensive services provided by the Chapa-De Diabetes Department. Year one activities will include a special focus on improving rates of completion and documentation of tuberculosis screening to prevent disease amongst AIAN clients and the wider community. In addition, the Chapa-De Diabetes Team will champion the use of Continuous Glucose Monitoring to help empower patients and allow providers to make customized treatment decisions for optimal diabetes management, partner with the Chapa-De Dental Department to identify AIAN adult patients at risk for diabetes through annual screening and offer them support and services, complete retinopathy screenings with adult AIAN clients with a diagnosis of diabetes and who have a Primary Care Provider at Chapa-De to prevent vision damage, and deliver Medical Nutrition Therapy by Registered Dietitians to support AIAN clients to build positive health behaviors and improve glycemic control.
