PROJECT ABSTRACT
The Jamestown S’Klallam Tribe proposes funding for $48,292 each year for the four-year grant to cover the costs of staff and other budget facets to promote Diabetes Education, primarily among those who live with diabetes. Based on our 2021 and 2022 audit findings, we have selected the best practice of Diabetes Education, with the intention of renewing healthy habits, giving insight into what other comorbidities may be a higher risk for our tribal citizens, and exploring together how to minimize complications associated with diabetes.
This grant funding covers the time specifically set aside for one clinic staff and one health department staff to focus on Tribal citizens with diabetes: 1) Donna Short, LPN, is our Tribal Nurse, and will be readily available to assist, educate and give guidance in diabetes self-care, regularly meeting with an exercise group at our Tribal gym; and 2) Ethel Colon, a Tribal citizen and our Community Health Representative, who will provide outreach to those who are not yet participating, offer group diabetes education events, and follow up with participants to encourage self-care and diabetes management through phone calls, home visits and written information. In addition to the grant-funded staff, Dr. Tom Locke, our Tribal Health Officer, will give input in the program development, occasionally giving talks at bi-monthly meetings with our group.
Our anticipated outcomes are a population of healthier diabetes patients, who make advances in establishing, or re-establishing good habits in self-care. Additionally, we hope for an increase in the number of participants in this program.
