The Cowlitz SDPI Project incorporates clinic- and community-based elements in an effort to enhance the prevention and management of diabetes in the tribal community. The project is administered by the Cowlitz Indian Tribe's Wellness & Diabetes Program, which functions under the auspices of the CIT Health Clinic. The main efforts involve diabetes education, population management, outreach activities, and community engagement. The main activities are as follows:
-Individual and group diabetes self-management education
-Remote physiological monitoring
-Professional continuous glucose monitoring
-Reminder cards and patient outreach for scheduling and follow-up
-Staff training and professional development
-Community wellness garden programing
-Cooking classes, grocery store tours, physical activity classes and cultural activities
-The Diabetes Prevention Program year long lifestyle change program
-Youth-focused initiatives, including opportunities for physical activity, healthy eating, and traditional food programming
