The Kanza Health Center is well aware of the growing prevalence of diabetes among Native Americans and of equal concern is the fact that Type 2, or adult-onset diabetes, is increasingly being discovered in our Native American youth. As of 8/15/2022 there are 2,698 established patients at the Kanza Health Center including 207 diagnosed diabetic patients. The overall vision of the SDPI grant is prevention and early treatment of Diabetes.
The SDPI program will offer a variety of services to its patients including: a) educational information concerning diabetes, its effects on patients’ health and lifestyles, and tools that patients can use to control/improve their health and lifestyles as diabetics; b) assistance in modifying patient treatment patterns such as the management of blood glucose and nutritional, weight control and exercise lifestyle patterns; and c) assistance with vision, podiatry and other diabetic healthcare issues including the provision of Diabetic shoes to patients.
The SDPI program will provide a consolidated effort, community outreach, and specialty services involving the following staff: Medical Doctor, Registered Dietitian, Podiatrist, Exercise Specialist, Pharmacist, SDPI Coordinator/RN, PHN, and related clinic staff.
