As outlined in the notice of funding opportunity (NOFO), the purpose of the Special Diabetes Program for Indians (SDPI) is to " provide diabetes treatment and/or prevention activities and/or services for AI/AN communities". The Confederated Salish and Kootenai Tribes (CSKT) Tribal Health Department (THD) has been engaged in SDPI programming for many years and we look forward to continuing this collaboration with the Indian Health Service (IHS). For this cycle of our Diabetes Program the THD has chosen to focus on diabetes-related education as our primary best practice effort. To meet the Required Key Measure (RFM) described by IHS, our main activities will include: health screening and education events, coordinated recruitment efforts in collaboration with tribal and non-tribal healthcare organizations, new and improved patient visit structures to accomplish comprehensive diabetes education, increased training and certification standards for our personnel, and robust continuing quality improvement (CQI) efforts for the program. Additionally, we will be implementing an integrated weight management program, expanding patient use and coverage of continuing glucose monitor (CGM) devices, and creating diabetes-specific eligibility for our compliance packaging ("bubble pack") service, among other standard diabetes best practice interventions (tobacco cessation, immunization rates, etc.) Please see the full project narrative and other supporting documents for additional details regarding personnel, location, and budget considerations.
