The Diabetes Prevention Program is a current grantee of the Special Diabetes Program for American
Indians Grant. The Colville Tribes Diabetes Prevention Program receives an annual $519,357.00 in grant
allocation from Special Diabetes Program for Indians. No tribal dollars. SDPI has not increased this grant
allocation in the last 10 years.
The Diabetes Prevention Program team of 3 health educators provide diabetes prevention education
services to 4 reservation districts and 6 local school districts on, or near, the Colville Reservation. The
health educators screen youth ages 9-18 with an electronic weight device, an “In-body” computer that
measures Body Mass Index to identify those at-risk for diabetes. Normal (<25.0) Overweight (25.0-29.9)
Obese (30.0 or above).
Determinates for eligibility for direct services from the Diabetes Prevention Program following SDPI
guidelines:
¿ Current Diabetes I or Diabetes II Prescription from a Medical Provider.
¿ Enrolled Tribal Member, or enrolled member of another tribe, eligible for Indian Health Services.
The disease of diabetes effects the entire physical well-being. The Diabetes Prevention Program
identified three primary health issues: vision, dental and feet to assist eligible diabetes clients with.
Diabetes clients suffer vision loss, gum and tooth issues, and foot problems. Our diabetes prevention
program provides financial supplements to eyeglasses for diabetes clients to assist with any vision
problems. Our diabetes prevention program provides financial supplements to dental cost to assist
diabetes clients with gum, tooth or any dental issues, our diabetes health educators promote healthy
eating. Diabetes clients tend to become sedentary resulting in compounded diabetes complications to
their physical health. Our diabetes prevention program assists with the fitting and purchase of special
shoes to aide in client mobility. Diabetes health educators encourage our clients to walk as much as they
are physically able. Once approved, each diabetes client is eligible for financial supplements for eye-
glasses, dental and shoe assistance one time, each fiscal year:

Summary: The Confederated Tribes of the Colville Indian Reservation Diabetes Prevention Program
provides Diabetes Prevention Health and Education services to enrolled tribal members. We collaborate
closely with the local Colville Indian Health Service Unit located on the Nespelem Government Agency
Campus, the Omak Health Clinic, and the Lake Roosevelt Health Center to assist tribal diabetes clients.
Our team of 3 Diabetes Prevention Health Educators provide diabetes prevention services to 4
reservation districts and 6 local school districts located on or near the Colville Reservation.
