The Little Traverse Bay Bands of Odawa Indians' 60-month Special Diabetes Program for Indians (SDPI) will promote diabetes education, care and prevention through the IHS Best Practice of Diabetes-Related Education. The required key measure is the number and percent of individuals in our target group who receive education on any diabetes topic, either in a group or individual setting. Diabetes-Related Education activities include Self-Guided Wellness Plans, Lifestyle Education, Nutrition Education, Physical Activity Education, Community Events, and the Home-Grown Project. Other activities/services not related to our Best Practice are use of traditional healers and youth-based diabetes-related education.
HHS’ Tracking Accountability in Government Grants System (TAGGS) website provides access to detailed descriptions of grants, loans, aggregated direct payments and other types of financial assistance awarded by the HHS Operating Divisions (OPDIVs) and the Office of the Secretary Staff Divisions (STAFFDIVs).
