This project, using Glycemic Control as its best practice allows our program to provide primary, secondary and tertiary care for the patients and community members of the Menominee Indian Tribe of Wisconsin. Intensive case management for the target group of 30 randomly selected patients with an A1C over 9 percent will be provided by program staff. The goal for the individual members of this target group is to lower their A1C to less than 8 percent and thus will improve their health outcomes. Additional support/services being offered through the project will include patient access to a podiatrist, a registered nurse to provide wound care if needed, optical services to detect retinopathy and a dental hygienist to support dental care for the target group. The podiatrist will be available two days per month to see diabetic patients. The remaining support specialists are regular employees of the clinic and will be available throughout the grant period. Diabetes patients are encouraged to access smoking cessation support, diabetes education activities and regular community-based exercise programs. The Diabetes Program will also focus diabetes prevention efforts on the youth in the community by providing after school physical activity programs. This after school program will also contain a healthy behavior educational component with topics such as diabetes prevention, eating healthy food, maintaining physical activity, not smoking, avoiding drug use and individual well being.
