Red Cliff Special Diabetes Program for Indians initiative provides pro-active supportive care to those living with and at high risk for developing diabetes. - The Red Cliff Community Health Center is a patient-centered medical home clinic. We have a wide range of services at the clinic to help care and treat our patients with diabetes. We also offer many preventive services to help reduce the risk of patients becoming diabetic. There is a lab on site which allows patients to be able to receive lab draws at their appointments with their primary care physician. We do contract with Podiatrist and Registered Dietitian, they provider services to our patients monthly. At the Red Cliff Community Health Center, we us a max packing approach at patients’ appointments which means when a patient comes into the clinic for any type of appointment weather for an acute or chronic issue, we make sure they are up to date on all their labs, immunizations, screening etc. The Red Cliff Community Health Center hosts many events throughout the year, a Health Fair and a Spring and Fall Fest. At these events there are educational tables, screenings such as A1c, cholesterol, healthy food option demonstrations, and physical activities. The Red Cliff Community Health Center has been receiving the SDPI grant since 2010, the grant has made it possible for the clinic to be able to offer many of the services named above.
