2023 Pueblo de Cochiti Special Diabetes Program
Pueblo de Cochiti has allocated a community directed Special Diabetes Program since its inception in 2001. The efforts and prevention services provided have made a significant impact on the awareness, intervention, and treatment techniques of the disease within the community. The number of diagnosed patients remains steadily based on the Indian Health Service diabetes audit.
Based on the recent SDPI Audit, Best Practices, Goals and Objectives, Activities and Accomplishments, and Needs Assessment the 2023 application will focus on Overweight, Nutrition Education, and Physical Activity. Due to the ongoing Covid-19 Pandemic, it has been highly requested of the program to provide weight management education, nutrition education, and physical activity avenues to the community. The Best Practices can be simultaneously addressed during events and educational classes via distance learning or in limited in-person gatherings. Due to the ongoing Covid-19 Pandemic, access to community patients has been limited to virtual classes or limited in-person meetings.
