Isleta Pueblo Diabetes Prevention Programs- Special Diabetes Program for Indians - The Isleta Pueblo Reservation is located 15 miles south of Albuquerque, New Mexico and covers an area of 329 square miles. The population consists of a southern ethnic group where the traditional spoken language is known as "Tiwa". The Isleta Pueblo retains its traditional beliefs and practices however, it has a heavy burden of diabetes. The number of patient cases is increasing and diabetes is the most common disease in the community. With a user population of 4,348 an estimated 16.5% of the population is suffering from diabetes. Diabetes Type 2 accounts for almost 100% of all diagnosed diabetes cases. The need to address diabetes continues to require urgent attention and the burden poses significant financial costs to prevent, maintain and treat it due various complications and variables. Over the years, the Isleta Diabetes Prevention Program has been successful in addressing the changing trends and the negative aspects diabetes poses on the community. The program is established with all the elements needed to effectively continue to instill positive health values to its targeted population. The program will utilize both process and outcome evaluation approaches to determine the effectiveness of implementation of services through the use of public health theory guidelines and evidence bases resources. Data will be analyzed to observe patient outcomes, address areas of improvement and report its successes on the required key measures. The target population will include at least 200 community members ages 14-75 with or at risk of diabetes. Programming will include medium intensive multidisciplinary approaches from both clinical and educational aspects of service. Diabetes Self-Management Education will be employed through one on one medical nutrition therapy, podiatry, specialized exercise plans complemented with an outreach of diabetes-related group interventions that align with culturally and socially appropriate methods of care and service.
