SPECIAL DIABETES PROGRAM FOR INDIANS - The grantee will provide diabetes education to the community of San Felipe Pueblo to assist them in managing their diabetes. The focus will be on the elder matriarchs, 60-65 years of age, within the household. We have identified that this target group prepares the food for the meals. We believe that if we can educate the meal makers to change their routines and prepare healthier options for their families there will be a cascade effect of healthier biometrics, i.e. weight loss, blood pressure reduction, cholesterol reduction and in turn diabetes management will improve. The community has communicated with us that diabetes is a "four-letter" word and programs or classes with the word in the name/title will cause individuals to shy away from attending. The SDPI program has planned to offer education through several lifestyle change workshops that include diabetes management but do not focus entirely on diabetes. We will continue to offer Medical Nutrition Therapy utilizing our contract Nutritionist/CDCES operating out of the village IHS clinic. Lastly, we will be implementing a weekly A1c and Abbott FreeStyle Libre (Continuous Glucose Monitor) clinic. The SDPI program has an open door policy for walk-in patients requiring assistance.
