The Pueblo of Nambe has provided Diabetes Prevention Program services since 1998. We are located 30 minutes north of Santa Fe, which provides 90% of our tribal members primary healthcare. The remainder 10% receive services at Los Alamos Medical Center (located 30 minutes west), Santa Clara Clinic (located 20 minutes southwest), and Presbyterian of Espanola (located 20 minutes north). Specialty clinics are usually done in Albuquerque which is located one hour and thirty minutes south of our pueblo. We provide transportation to primary and specialty medical services. It takes on average 2.5 hours per dialysis patient for their transportation. We have the following food sources located within the pueblo: Eight Northern Indian Pueblos Council, Inc. Food Distribution Program and a community farm. A grocery store and farmer's market is located 15 minutes west of the pueblo. Program Manager has worked with the Pueblo of Nambe since 2002 as a Community Health Representative (CHR) and Diabetes Prevention and Education. She has been trained in Diabetes Education through the Santa Fe Community College and provides one on one home visits to help ensure all diabetics/pre-diabetics needs are met and advocate for them if needed. She also provides community wide activities that include physical activities, education, screenings, and resources. The SDPI Program will continue to work on the following; After-School and Summer Youth Program to provide minimum of 30 minutes of exercise a day and daily healthier food consumption. Engage the youth in the Coordinated Approach to Child Health (CATCH) Program. Foot and nail care for diabetics and elders that request services. The SDPI Program works closely with our client's doctors. We keep charts on each client with their medications and services provided. We are allowed to schedule appointments, and follow-up between client and provider, and work as their advocate for their healthcare services. We have do
ne chart reviews and are part the Patient Centered Medical Home (PCMH) through Indian Health Services. Provide community newsletters that contain physical activity education, health information, and community activities. Provide physical activity trainer certifications. Provide physical activity classes. Host monthly community activities that promote physical activity, physical activity education, and healthier food consumption. Provide Group Lifestyle Balance Classes and other classes to help with mental well-being.
