The Jicarilla Apache Nation Health & Fitness Center intends to focus on Diabetes-related Education as a Best Practice and as identified within the Jicarilla Service Unit Diabetic Care and Outcomes program. The main outcome we anticipate achieving is to decrease our client’s A1C's and BMI's and increase their understanding of healthy lifestyle choices and practices. In 2022, the Jicarilla Service Unit Diabetic Care and Outcomes Audit identified 268 tribal members with diabetes, of those, 52% were in the category obese, with 17% having an A1C over 11.0. Based upon additional data, only 64 out of the 268 individuals with diabetes received some level of nutritional education with a Registered Dietitian. Our plan is to both provide nutritional education for our program staff, which will help to bridge potential gaps in communication, and supplement the nutritional education and other preventative services provided by the Indian Health Service (IHS) Jicarilla Service Unit (JSU) medical providers who sometimes have time constraints related to educational services for clients. The SDPI staff will meet with individuals, one-on-one and at their current level of understanding, to provide continuing education and support needed to obtain their health goals in the healthiest manner possible. SDPI staff will also provide group and individual physical activities to our community and an identified Target Group, which will focus on specific ages and health status to improve our client's BMI's and A1C's.
